COA Coastal Home Care and Coastal Care Management

COA Transportation Services
This door-to-door (para-transit) service is funded through the Florida Commission for the Transportation Disadvantaged and is available to those who have no other means of transportation due to age, income or disability. The service is also available to transportation-eligible Medicaid recipients. For Customer Service, call (904) 209-3710.
Sunshine Center
The Sunshine Center Therapeutic Adult Day Center provides supervised care with structured activities for those who can no longer be left unattended. Aside from providing cognitive awareness exercises within a caring atmosphere, the Sunshine Center also offers much needed respite time for caregivers and allows family members the option of continuing their individual work and recreational schedules. The Sunshine Center is open Monday through Friday from 7:30 am until 5:30 pm.
Licensed nurses and specially trained staff are available to assist with activities of daily living and personal care. A nutritious lunch and snacks are served each day and a regular schedule of exercises and daily activities are planned and supervised by a staff of trained professionals.
Activities include: Songs, Music, Dance; Arts & Crafts, Horticulture, Bingo & Table Games, Active Games, Daily Exercise Program, Cognitive Stimulation, Outdoor Activities, Parties, Celebrations, Field Trips and Sensory Stimulation .
